BED WETTING
------------------------
It is a common problem where children pass urine in the night from the day they are born. It is considered abnormal if the age is more than 5 year.
❇️ *Causes of bedwetting*
There is no specific cause for Nocturnal enuresis, but few factors may trigger bed wetting at night.
🔹 Hereditary
Either parents or grandparents have such problems during their childhood.
🔹 Sleep
If the children are having following sleep problems.....
▪️Snoring while sleeping
▪️Disturbed sleep due to television or mobile
▪️Deep sleepers
🔹 Medical diseases like Diabetes, Urinary tract infection, chronic constipation.
🔹 Bladder dysfunction
▪️ Small bladder capacity
Such children passes urine frequently during whole day even in the school
▪️Poor emptying
Such children are not able to completely empty the bladder in the day which results in excess urine being stored causing bed wetting in the night.
▪️ Insensate to bladder
Sometimes, the nerves which control the bladder are not completely developed in a child. This results in the child not being able to feel that the bladder is full or get the urge to urinate. This results in the child leaking urine or having Nocturnal Enuresis.
🔹 Diet
Food rich in salt or caffeine like kurkure, chips, chocolate, coffee or even Tea
🔹 Making too much urine in night
This excess production at night is due to low levels of antidiuretic hormone.
🔹 Stress in children
There are a lot of causes for stress in children and teenagers, few common causes have been discussed here....
▪️ Parental conflict
▪️ Difference of opinion
▪️ Poor discipline technique
▪️ Over ambitious parenting
▪️ Comparison
▪️ Bullying
▪️ Fear of failure
▪️ Set back
In short... All these factors are related to parenting
❇️ Investigation
Before starting treatment, do the following investigation...
▪️ Urine examination
▪️Ultrasonography
❇️ Treatment
🔹 Treat the cause
🔹 Don't feel ashamed
▪️ Maximum parents feel ashamed if their child passes urine in the night thus children feel guilty.
▪️ Don't blame or punish the child for this problem as there is no fault from their side. Actually, it increases stress, feelings of shame & teaches kids to hide their bed wetting.
🔹 Lifestyle modification
▪️ Avoid food after 4pm which increase urine production as discussed above
▪️ Finish dinner 2 hour before sleep.
▪️ No liquid including water after dinner
▪️ Encourage children to pass urine before sleep. Better to pass urine in a squatting position so that he can empty the bladder completely.
▪️ Affirmation
Before sleep, ask the child to repeat that he is not going to pass urine today's night. It might work.
▪️ Wake up
Any family member who sleeps late can wake up a child for passing urine.
▪️Ask a child to clean the bed if he or she passes urine.
🔹 Reward therapy
▪️ Make a book for 1 month and ask the child to write whether he had passed urine or not.
▪️ Promise the child to reward him if he had not passed urine 5 times in a month. If he or she has achieved a desired goal, immediately fulfill the promise.
▪️ Now increase the target for 10 day in a month with a bigger reward.
🔹 Alarms
Bedwetting alarms typically clip to your child’s underwear or have a pad for your child to sleep on. When the clip or pad gets wet, the alarm sounds or vibrates, which teaches your child’s body to wake up when they need to pee. In order for this to work, your child actually has to wet the bed repeatedly so their brain can learn to wake up to pee. Research shows that, when done correctly, it is the most effective method of teaching children to stay dry in the long term.
